Please make all checks and money orders payable to Rev. Dawn Halstead and mail to: Rev. Dawn Halstead, Ceremonies of Celebration, 4734 76th St., Urbandale, IA 50322. OFFICIANT RESPONSIBILITIES
Officiant agrees to provide the following services for the full ceremony under the terms of this agreement.

  • Initial consultation
  • Consultations by phone and/or email with Couple to discuss ceremony details.
  • Workbook, A Wedding Ceremony to Remember, to help Couple personalize their ceremony.

  • Performance of wedding ceremony to Couple in a professional and respectful manner, on the date, time and location of ceremony listed within this agreement and in accordance with the ceremony planned by the Couple.

Officiant retains the right to edit the ceremony in case of redundancy and/or inappropriate wording. Officiant reserves the right to refuse any service that she is not comfortable performing, at which time a partial refund may be made to the couple.

COUPLE RESPONSIBILITIES

  • Couple agrees to be available to the Officiant for whatever purpose deemed necessary for the completion of the written ceremony.
  • Couple agrees to fill out the Ceremony and Rehearsal worksheets from A Wedding Ceremony to Remember, and vows, if writing their own, to the Officiant no later than 2 (two) weeks prior to the service.
  • Couple agrees that if a rehearsal is not requested when booking, a rehearsal is not guaranteed if requested at a later date. (Officiant will make every effort to work with couple to find a time to hold a rehearsal.)

  • Couple agrees to not post negative reviews of Officiant on the Internet or elsewhere or to make negative verbal or written statements about Officiant until all efforts to resolve said discrepancy have been exhausted.

MARRIAGE LICENSE
For a legal marriage to take place, Couple must obtain a valid marriage license in the state where the ceremony will take place. The marriage license needs to be presented to Officiant at least 15 minutes BEFORE the ceremony. Failure to have a valid license means that Officiant cannot perform a legal marriage ceremony and is under no obligation to do so. Officiant may at her discretion perform a symbolic ceremony, which will have no legal merit, and perform another ceremony in private for the couple once the legal documents are presented, for an additional fee of $50.00. If couple fails to bring marriage license to the wedding, there will be a fee of $25 for meeting with officiant at an agreed upon time, to obtain signatures.

Two adult witnesses also are required to be present. Officiant will mail the license to the appropriate clerk's office within 2 business days of the wedding ceremony.

CEREMONY AND REHEARSAL TIMES AND OVERAGE FEE
Couple and wedding party must arrive 15 minutes before the scheduled rehearsal time. Officiant allows 45 minutes per rehearsal. If more time is needed due to a late start, and if Officiant is available, then a fee of $25 per each additional 15 minutes beyond the time allotted will be charged until the rehearsal is finished. Couple understands that the overage fee must be paid immediately after the rehearsal is concluded.

Officiant and Couple agree to arrive at the wedding location listed on the contract at the agreed upon time for Couple's wedding ceremony. Couple understands that they must adhere to these times. Weddings do not always go as planned, but Officiant can accommodate weddings that start (or end) later than scheduled only as Officiant's schedule permits. Officiant allows for a 1 (one) hour block of time per wedding to give Couple plenty of time and give room for unexpected delays. If more time is needed, and if Officiant is available, then a fee of $25 per each additional 15 minutes beyond the hour allotted will be charged until the ceremony is finished. Couple understands that the overage fee must be paid immediately after the wedding is concluded. Officiant cannot and will not be late for any wedding booked after Couple's. This applies to Couple's wedding as well, except for extenuating situations listed above. Overage fees are waived in the event of an actual emergency. An emergency is defined as a catastrophic event or act of God.

DATE CHANGE BY COUPLE
If Couple needs to change the wedding date or time, and Officiant is available on the new date, Officiant will simply change the date and re-reserve it under the new date. If Officiant is unable to officiate the ceremony on the new date, and consultation has taken place, and Couple has received a draft of the ceremony, the full contracted amount is due to Officiant (minus travel and holiday fees). If Officiant is unable to officiate the ceremony on the new date, and consultation has taken place, the booking fee/deposit is forfeited and Officiant is relieved of all responsibilities set forth in this contract.

Couple MUST contact Officiant immediately if there are any changes in rehearsal/ceremony times or locations! If Couple fails to communicate changes and Officiant arrives according to contract time, Couple is still responsible to pay for Officiant's services.

If Couple changes the time or date of the scheduled ceremony without Officiant ‘s consent, Officiant reserves the right to cancel her performance of or participation in the ceremony, and shall in no way be held responsible or liable in any manner for such non-performance.

CANCELLATION BY COUPLE
If for any reason Couple decides not to use the services of Officiant after payment of the non-refundable deposit, Couple must inform the Officiant, in writing, at least thirty (30) days prior to the wedding date in order to release Couple from this contract. If Couple properly notifies Officiant of cancellation after Couple and Officiant have had a consultation and a draft of the ceremony has been presented to Couple, 80% of the total amount of this contract (minus travel and holiday fees) must be paid to Officiant. If Couple properly notifies Officiant of cancellation before the ceremony draft has been presented to Couple, but after consultation has taken place, 70% of the total amount of this contract must be paid to Officiant. If Couple notifies Officiant of cancellation before consultation has taken place, the Couple will forfeit the 50% non-refundable deposit/booking fee, not as punishment, but as compensation for a breach of the Couple's obligations under the contract upon which Officiant relied.

If Couple cancels the wedding or Officiant's services within 30 calendar days of the contracted wedding date, Couple agrees to pay the full contract fee (minus travel and holiday fees) to Officiant.

OFFICIANT UNABLE TO PERFORM
In the unlikely event that Officiant is unable to perform the ceremony due to illness, hospitalization, family emergency, transportation breakdown, or any other unforeseeable causes, Officiant will make every possible attempt to notify the couple, and will attempt to find a substitute Officiant who will provide the same level of service. In the unlikely event that a replacement cannot be found, then Officiant will make a full refund of the fees pre-paid by the Couple and Officiant is relieved of all responsibilities set forth in this contract. Couple agrees that the refund will be the full extent of damages they are entitled to, and Officiant or any persons affiliated with Officiant, will NOT be held liable for any damages (including punitive) due to the non-performance of the ceremony.

TERMINATION CLAUSE
Disruption of the ceremony due to unruly actions or disorderly conduct from Couple and/or guests is a breach of contract and may incur termination of services. Couple agrees that Officiant will NOT be held liable for any damages (including punitive) due to the non-performance of the ceremony in these circumstances, and that Couple is not entitled to any refund of fees already paid to Officiant.

PHOTOGRAPHIC RELEASE
This contract serves as release for any photographic likeness taken during the rehearsal or ceremony to be used in print or electronically for promotional purposes only and guarantees they will not be intentionally exploited in any way.

AGREEMENT AND GOVERNANCE
This agreement is valid for the wedding service for the place and date stated above. All parties agree that this agreement and any attachments constitute the entire agreement between Couple and Officiant relating to the officiating of the ceremony, and may not be modified except in writing signed by both parties. No other representation or promises have been made except those that are set out in this agreement stated above.
